I read that the lungs absorb the thc in around 3 seconds. (I think, it's been a few years since I read it.) Anything past that is just oxygen deprivation.
Finally, something 'scientific' ....or, at least a fact. I can hold it for 3 seconds without coughing.

The interesting thing is that i would like to see the difference it makes, however with my illness I can NOT tell the difference between smoking some grown from 'bag' seed compared to all the different strains I've purchased seeds to grow and paid a lot for. I read all the different ways it's supposed to make you feel, but it's all the same to me. Except only ONE time when I bought some that was some killer weed (don't have any idea of the strain, though), and I couldn't even add the dots on dominoes to play double fives! I was SO out of it...but it was the ONLY time I've noticed a difference....and it really cost a lot. I can't seem to find any seeds that are that good..;but then again, I'm not looking to get stoned, just help some ailments....and, so far it isn't helping the pain, just the nausea. I'm still searching for different strains, and inflammation is supposed to be the best for 'pain', but I still don't notice any difference. I'll keep trying different strains. Who knows, maybe one day I'll find one that helps the pain. i hear of people smoking for pain all the time....gotta be something out there to help.

And, 'what we are holding' is the smoke we take a hit off of.

We are holding the smoke in to see how well it works by how long (or not) we hold it in.

In early days I was lucky to have a 1/8 so I held the stuff in as long as I could. Now I figure the moment it hits the lungs its been absorbed and I don't need to be doing anymore damage to my lungs. Its also nice to have a variety pack of delivery methods available. I rotate through a pipe, bong, flower vaporizer, edibles and tincture.

You don't need to hold it at all and considering you have COPD you don't need any more alveolar issues. The walls of the mouth, esophagus, bronchus, bronchi and alveoli are very vascular, massive in area and will absorb readily. You get nothing but more O2 deprivation by holding for any length of time you need less hypercapnia. It's counterproductive for COPD patients so please don't do it.

However if a hobbit tells you something different I'll defer, good morning and Merry Christmas.

I hold it with two fingers, unless I'm using the paper from the Big Bambu album.
An old head I worked with in the ‘90s told me about a gigantic spliff he once rolled with the paper from that Cheech and Chong album.
Said he put about 4 lids of Mexican in there - it took about 2 hours to de-seed it all first. Thing got passed around at a party for the better part of an hour and got a bunch of people baked. Not sure how long they held it their hits though.
We used to try to hold in 3-foot bong hits for as long as we could and all turn red in the face. The coughing fits were sometimes epic. My lungs can’t take that abuse these days, plus the science proves we were just being dumb shits doing that.

"The longer BHT (Breath Holding Time) significantly increased both percent retention of tar in the lung and the pre- to postsmoking rise in blood COHb, serum THC and heart rate, independent of puff volume and number. In contrast, the larger PV (Puff Volume) had no significant influence on these variables for the same amount of cigarette consumed. The longer BHT (and not the larger PV) characteristics of M smoking contributes to the greater COHb boost and lung retention of inhaled tar during M compared to T smoking. In addition, the longer BHT appears to enhance THC absorption."
