From what I've read here, hole should be 3'x3'x3', spaced 4' apart at least and not in lines to avoid aerial detection Also ^ seems to recommend a 2'x2' hole, so either one, 3' would be better but more of a pain obviously.

Re-potting should help, something a little larger & new soil. I'd use a little bit less perlite, 15/20% tops, but that's just me.
