That looks like start of a nanner. They look a little heavy on N. Nanners and overfeeding goes hand in hand. Some plants don't develop them at all. Some plants are keepers and some are not.

Looks like your doing almost everything thing right. Your plants dont look to have suffered any serious stress problems, they look healthy. You shouldn't be getting this same issue happening every time with your minor ups and downs that most other people go through aswell who dont get the same problem. Its got to be those genetics. Try it with afew different seeds from a different breeder next time to see if there is a difference.
