How do you top a plant multiple times?

Hello this may seem like a stupid question. But I wanna know you top a plant multiple times? I know how to top plants once. But if you top again, are you still topping from the top of the stem again or are you topping side branches?

After topping the first time you will have two leaders. I let them grow a few nodes, then top again to grow four leaders. Top each of those for eight leaders, etc.
