Where to get forms?

I am starting a small grow for me and a couple of friends.. We all have our med cards here in so. cal. Since I am doing this at my home I am wondering if there is a web site or someplace I can down load a generic form of some kind stating that I am growing for them, because I will have more than what would be considered legal for one person.. Thanks for any direction anyone can provide to me.. PEACE

Hello, I do have all of the available forms for the state of Colorado.

These include:

  • New Applicant Form over 18 YR
  • New Applicant Form under 18 YR
  • Parent Consent Form
  • Change of Caregiver/Address or Name Form
  • Physician Certification Forms
  • Heath and Regulations of MMJ
  • Copy of Amendment 20 (Most Important)

Check out the NORML webpage, the form you want is for Primary Caregiver. It's actually just a personal contract so there is no precise wording nedcessary, technically if your friend wrote on a piece of scrap paper "I assign _____ my primarty caretaker" with signatures and dates it would probably be good enough. But print out the form from NORML anyway cause it looks official and cops are dumb (like all people).

its important to know how you will doing it, less on the application because you already have the neccessary paperwork (mmp card) ... being a "caregiver" doesn't necessarily give you the right to grow for other patients or members, unless if you provide consistant care and meet other criteria designated under the CA Attorney General Guildlines...
