T5 replacement ballast, which one do I need?

I have a 6 bulb HO T5 fixture, it was given to me so warranty is not an option. Two bulbs went out about one month apart from one another. I've tried other working bulbs in their place but nothing has worked. A friend assured me that it was a ballast problem. One ballast powers two bulbs and the other ballast powers four bulbs. I'm not sure if the problem is with one or both ballasts. Included pics of the ballasts' labels.

Could someone with experience recommend a compatible icecap or icehorse set of ballasts?

I would assume if you open the fixture there should be a lable on the ballast saying what it is.

I did open the fixture. The image links are pics of the labels on the ballasts. I'm just not sure of exactly what I need to be matching up with the replacement..

Just replace the 4 bulb ballast if that's it? I just went to homedepot and got one for $40 Phillips ballast will last very long. ebay got's t5ho for cheap. Also buy a pack of sockets too.

What specs do I need to match I guess is what I'm trying to get at....

Besides voltage and the fact that it's for a 24" t5 hO?
