Ps form 4232 help please

Hey everyone. I ordered seeds from nirvana about 6 days ago and received this notice in the mail from UPS...http://www.usps.com/forms/_pdf/ps4232.pdf. I moved from my house about 5 months ago so i don't know if there is any relationship. Should i be worried? Or fill this out and send it in?

I did do that. I hear of people doing it all the time and getting away with it. Besides whos name would i use and i dont know where else i could send it. Shit man should i be worried?
