Need some medic paperwork advice

i just moved to colorado and i want my ganja papers. the only worry i have is that because of my personal beliefs i can not accept modern medical treatment and because of this i have very little to go on as far as medical history. i do have chronic back problems among other pains, should i be ok if i just tell the doctor about my pain?

i just found a clinic in denver online whilst looking for a place to talk to a phd and they said that no records are needed, denver is a bit far but this tells me that it must be practice not law.

Contact these people and ask about all that... Im in the process to moving to denver and they said they can hook me up
