Leaving my caregiver to grow for myself, what needs to be done

I am a little confused on this and after searching for a couple of hours i have yet to find anything resembling an answers. Heres the deal, my caregiver quit growing my meds. I want to remove him as my caregiver and grow my own plants. I guess the question is as follows, how do I fill the section out

Section A: Patient information. Obviously this is my information. not a problem here.

Section B: Changes reguarding Primary Caregiver
(options)
1. add change caregiver
2. no change in caregiver
3. caregivers address change
4. remove caregiver

On this section im not sure which option to choose, Im assuming it is add change caregiver. I dont know what paperwork or documents I need to send in pertaining to this section

Section C: Person allowed to posses patients plants
(options)
1. no change in person allowed to posses plants
2. change to new caregiver
3. change from patient to current caregiver
4. change to patient

Again im not sure what option box to mark.

Only thing I would really come up with is fill out the paperwork as follows.

Section B: select (option) *add/change caregiver* then fill out the lines with my information to become my own caregiver.
Section C: Select (option) *change to new caregiver* Which this caregiver would obviously be myself. Since I filled out the

Section B would be 4, and section C would be 4 as well.

Someone correct me if I'm wrong .... but that language seems self explanatory. You're removing your caregiver, and designating yourself (the patient) to grow the plants.

Thats How I have it filled out now. So I leave section B blank... or do I need to send it in with my current caregivers information and photo copies.

In order on the app it goes like this:
A. Your info

B. Change regarding caregiver check the "Remove caregiver"


C. Check "change to patient "

section C. notation "if a new caregiver is added and no box is checked above, plant possession will default to the applicant/patient."

E. Sign and date it.


They refer to the applicant "you" as the patient..

With all the money this state is saving on not putting any salt down on our ice covered road they could hire a few people to run a help line for the mmj program or at least outsource it to some broken english indian out east.

im going to do it that way and cross my fingers, im just gonna leave the caregiver part blank. ill let everyone know how it goes in a few weeks.

misunderstanding, im checking the "remove caregiver" box but as far as the information aspect "name, address, social" im leaving that part blank.

heres how i filled it out

Section A: my info "name, address, social, birth date"

Section B: Checked "Remove Caregiver" box. Left the rest of the lines blank "name, address, social, birth date"

Section C: Checked "Change to patient" box

Good luck OP.
Keep everything simple but your lighting, go big on the lighting! You can find great tricks and techniques on most the other stuff.

my op is already setup, just changing my card back to my house and getting rid of a patient that is a pain in the ass. the caregiver who had my card doesnt want to grow anymore, he started a small business and said that he doesnt have the time anymore. you can see my grow in my sig, I havent updated it in long time though lol.
