Ive never seen this happen. Help !

First pic is first sprout and 2nd is 2 days later. I have no idea what is going on here. Any ideas ? Im not an expert grower but my current autoflower is doing just fine at 5 weeks

I can't see what's going on, your pics are horrible lol.
Next time please use natural lighting.
Sometimes the thin inner layer of the seed gets stuck on the sprout, go examine it and see if that's what's happening, if that's not the problem we need better pics to help you.
 
Shell is off but the cell membrane is still on? I use tweezers to hold the baby stem in place and use nail clippers to gently pull it off. I'd rather 2 pairs of tweezers but i only have one. I don't squeese the clippers, just use the sharp pointy edge to pull the skin off. No harm, no foul.
 
Yep. It happens sometimes. You can get in there with something small and sterile. I just use my pruning scissors. Sometimes you can kinda just scrape the edges of it and itll open, but other times I've had to carefully cut it open by snipping off the very edge where they are stuck together.
Cant stress it being clean enough.
 
It looks kinda limp like an over watered sprout. A strong seedling with a bit of humidity has the strength to break through the seed and spread its wings. Get them some air flow and up your temp.
 
I can't see the membrane, just looks like it tapped out. Don't water at this stage and if its cold its going to struggle.
