How does heath get inside the grow to do things in the vertical set up? does he use a ladder or something?

Nice , I run one similar to this in addition to air cooled hoods .. My fan does the trick as well if positioned on the floor like yoursthere is a fan on the floor in the middle blowing straight up at the bulb.

the only problem with running the open bulb with the fan blowing up is that it blows the hot air through out the whole room making the whole room warmer not just the ceiling.

Unless you have the extraction just above the lamp so the hot air is blown upwards then extracted.
I agree with Zamzia, built the extraction right into the reflective hood that is put over the grow. Think of it like the fume hood over your kitchen stove.
I like these ideas for a bedroom closet. I'm picturing double doors with the plants attached to the walls and the doors with the ability to open the entire thing and work inside it.
