I'll tell you my side of the story. I'm a cyclist, I only travel by bike. And even though I roll (30-40km/h) I respect other drivers and I deal with them as a driver should, IE not assuming everyone knows how to drive.
How it works is I come up to STOP sign and I see there's a car that will go across. So I brake and STOP at the STOP sign. As I should.
But the driver, also stops, and makes a hand gesture for to go across. So I just make a hand gesture back to tell them to go ahead. It usually goes back and forth a few times until I get tired and just go through.
Many times, drivers let me pass when they shouldn't...
So nowadays, when I see a stop and it's SAFE (looking both sides twice) I just go through instead of risking trying to "argue" with the driver.

So in retrospect everyone's at fault somewhat (although I'll give you that there's a lot of people that don't know how to ride respectfully). Polite riders will use eye contact and hand/arm signaling ;)
