My card expiring... Is renewing any different?

Im currently hard up of funds and unfortunately I wasn't able to use my card as much as I would have liked before. I went to CannaCareDocs in south Boston to get my card 2 years ago and I see that now they have a location in seekonk which is way closer to me. Does anyone know if they are still operate in a similar way? Also I'm confused as to which application I need to use. The renewal application the ri DOH sent me says it is for patients using the same practitioner as before (which is prob not going to be the case with myself) but on the new patient form it says if you have been a licensed patient before and are renewing to not use that form. So they contradict each other. Does anyone know which form I would use?

I used the form they sent me in mail. When i went to seekonk office they gave me forms to fill out and he signed me forms the state gave me to be mailed in.

Long story short, forms state sends need to be signed and youll be fine.

Forms they have you fill out at doctors they keep for themselfs.

I had diff doctor this time cause went to seekonk vs boston 2yrs ago also and they didnt question it at all for me. Mailed me new actual id card 3-4 weeks later

Ok thanks for the info Ricky! Do they have certain days they see RI patients like they used to at the south Boston office 2 years ago at the seekonk office? Or can I just stop by during reg operating hours.

Was it easier because you were renewing instead of getting a first card Ricky? I don't want to go get tons of medical records if I don't need to.

I got my card a few months ago, but I've been wondering about this.. I remember it being a significant point in the application process (asking if I had already been a patient)

Also, is there any chance of my card expiring before I receiving a new one? (assuming I stay on top of things, and get the paperwork done when it comes in of course)

U can go any day they take ri and mass patients together. Just stop in during regular hours. Good luck.
