Two questions

Is my light blown 250w mh and whats this plant problem, im using standard well water and tiger bloom.

I couldn't tell you whats wrong with your light, maybe its old?

Is that MH new? and what wattage ballast are you using?

As for the leaf it could possibly be a genetic defect.

cut back on the nutrients looks like that maybe and i say maybe to much right now for such a tiny girl. cut back and see wht happens over the next few days. and for your light I dunno is that a pic of it lit ? If its lighting but not starting it may be a ballast problem, But with the Mh it shouldn't need a starter I dunno but i do have a 600 mh I'll would give you i am not using anymore. anyway she is small to be giving to many nutes. at that stage. It may have got water with the tigerbloom in on the leaf and could of turned it. be patient she should come back with minimal nutes when its is that size. Good luck.
