Have u ever seen or heard of this problem

This is the full plant

So I planted it and it's 3feet talk by 2 and a half feet wide it's been going great but like last year coming down to the last 2 weeks 2 buds and leaves attached to the same stem look like this it has never spread more then 3 but does anyone no what causes this?

It doesn't affect all the buds maybe 2 or 3 max

2years in a row now my plant has had this happen 2 weeks before harvest I wanna no what the problem is... If anyone knows or has any ideas that be great thanks

5 days from now it will be a full 4 weeks of growing i have done nothing to the soil except water them with my hose and put some old eggshells should i be doing more?

ive heard that they need 12 hours darkness and 12 hours light till they start to bud? and when will I no when they are ready to veg?
