It has happened to me, but in my case the cause was a nutrient disbalance, specifically an excess of ammonia nitrogen, paired with potassium deficiency. Some strains are more sensitive than others. Not sure if that's your problem tho.

Nitrogen excess is a likely cause I've had several plants like those you describe, and all were my fault. Believe me, a nitrogen excess ni bloom can cause the shit you describe.
