The doctor did it for me but they didn’t end up sending it in until 4/17. No idea why it was nearly 3 weeks later that they sent it in.

Doctor: 3/28 Paperwork Submitted (by doctor): 4/17 Issued: 6/26 Received in mail: 7/5 I did not receive any notice that I had been approved. I also never called or e-mailed, just waited.
