Dying branches

I have Some outdoor plants that are in the ground and there about up to my belly button so far. some days ill lose a branch it will be wilted and fall off easy and when i open up the branch it will be a lightly brown and black on the inside ill upload pics when i get the chance any body know what it could be just off what i described ?

cant figure out what it is and its on another little branch was thinking its a worm but cant figure it out its only affecting one of my plants at this spot so just trying to get rid of it

Looks like downy mildew aka stem rot might want to remove that plant once its in a plant it moves around fast. I do not know of a cure just some preventives.

Maybe make up some good aerated compost tea and spray the plant with it?
Jorge Cervantes says that the compost tea outcompetes everything else, so it might kill off the bacteria or mould that has got into your plant.

Ah i only have 2 plants in this spot so dont think i wanna cut it down just yet. Im thinking its the soil not draining enough so i may just let it be and try and use sum hydrogen peroxide as a mix and see if i can stop it from spreading.

And how would i make that tea im all ears right now would rather try something then lose a plant

Just get some compost in a plant pot and run water through it, and collect the 'tea' that drains out of the pot.
Then put an air stone in the tea (air stone attached to an aquarium pump) and aerate it for 24 hours or so (you could just put it in a bottle and regularly shake it to aerate it but it won't be quite as good, but still useable).
Then when you are ready, mix the tea with some water and spray all over the plant.
I don't have an aquarium pump so at the moment I have compost tea in a bottle and I shake it up to aerate it every now and then.
You could add a little blackstrap molasses (black treacle) or brown sugar to the tea to liven up the good bacteria and fungi in the tea, but it's not essential. Maybe give the plant a good spraying with it a couple of times over the next few days and see if it remedies the issue.

Do u think i should also water it with this "tea" to help the roots out aswell?
